Effect Of Different Positions On Various Vital Parameters Of Newborns Admitted In Nicu

Abstract :

Aim: To study the effect of different positions on various vital parameters of newborns admitted in NICU.Methodology: This prospective observational study conducted in the department of paediatrics at Shri Guru Ram Rai Institute of Medical and Health Sciences (SGRRIM & HS), Dehradun, Uttarakhand during the period of one year. A total of 300 neonates admitted in N.I.C.U. were placed in 8 positions, namely prone 0(P0), prone 30(P30), supine 0(S0), supine 30(S30), left lateral 0(LL0), left lateral 30(LL30), right lateral 0(RL0) and right lateral 30(RL30). Oxygen saturation, heart rate and respiratory rate were monitored and recorded after 30 minutes in all 8 positions. The data obtained was subjected to statistical analysis (SPSS version 17) using paired t test and anova test for various intragroup and intergroup comparisons respectively.Results: The mean oxygen saturation was 96.161.53, 96.161.6, 95.021.72, 94.331.98, 97.221.56, 96.521.53, 94.721.84 and 94.482.07 in P0, P30, S0, S30, LL0, LL30, RL0 and RL30 respectively. Similarly the mean heart rate was 143.367.47, 143.297.22, 146.957.12, 149.897.34, 140.257.43, 142.087.33, 148.437.06 and 150.277.44 in P0, P30, S0, S30, LL0, LL30, RL0 and RL30 respectively. The mean respiratory rate was 53.037.04, 53.357.04, 56.207.02, 58.5527.19, 50.536.75, 52.276.91, 57.337.11 and 58.277.27 in P0, P30, S0, S30, LL0, LL30, RL0 and RL30 respectively. The difference in the vitals was statistically significant (P<0.001) among four positions with 0 and 30 degree elevation. But the difference in the vitals was not statistically significant between right lateral 0 and right lateral 30 degree.Conclusion: The prone 30 degree position favoured better vital parameters than other positions.
